Duties and Responsibilities

• Provide one-to-one nursing care based on patient-specific treatment plans.
• Monitor patient's health condition and record vital signs, reporting any changes to the supervising RN or physician.
• Administer medications and complete treatments as prescribed.
• Assist in the development of care plans and participate in team meetings to discuss patient progress.
• Operate and monitor medical equipment.
• Perform routine nursing tasks like dressing wounds, administering injections, and inserting catheters.
• Offer education and support to patients and their families about managing illnesses or injuries.
• Ensure the home environment is safe for the patient by identifying potential hazards.
• Collaborate with other healthcare professionals to provide holistic care.
• Maintain patient confidentiality and records securely and accurately.
• Prepare reports and correspondence as required.

Requirements

• Current and valid LVN license in CA.
• 0-2 years of nursing experience; new graduates are welcome to apply.
• Experience in home care and pediatrics preferred but not required.
• Ability to work part-time across 2-3 shifts per week.
• Strong observational skills and a detail-oriented approach to patient care.
• Excellent communication skills and the compassion necessary for effective patient and family interaction.
• Physical endurance to perform job duties efficiently throughout 8-hour shifts.
• Demonstrable commitment to continuing education and professional development.
• Keen understanding of health and safety guidelines and the ability to implement these in a home setting.
